COMMISSIONED LAY PASTOR FORM

The Book of Order states that the commission of a Commissioned Lay Pastor "shall be valid for a period up to three years as determined by the presbytery. It may be renewed at expiration or terminated at any time at the discretion of the presbytery. Presbytery shall regularly provide resources for the person's spiritual and intellectual development. A review of the work of the commissioned lay pastor shall be conducted annually." (G-14.0801b)
In order to assist the Committee on Ministry in the annual review of Commissioned Lay Pastors, please respond as completely as possible to the following questions.
